Service Level Agreement For Hospital It Integration Services

Uptime Guarantee

The Provider guarantees a minimum of 99.9% Uptime for the IT Integration Services during Service Hours, calculated on a monthly basis. Scheduled Maintenance periods are excluded from this calculation. The Provider will provide at least [Number] ([Number]%) advance notice for any Scheduled Maintenance.

Service Credits

If the Provider fails to meet the Uptime Guarantee in any given month, the Client shall be eligible for service credits as follows:

  • Client's misuse or unauthorized modification of the IT Integration Services.
  • Scheduled Maintenance.
  • Client's failure to cooperate with the Provider in a timely manner to resolve an incident.

Reporting and Escalation

The Provider will provide monthly reports detailing incident response times, resolution times, and monthly uptime percentages. An escalation path for critical incidents is as follows:
